How do I request excusal for a medical condition?

If you have a medical condition which precludes you from serving as a juror, you must submit a statement from your doctor on the doctor’s prescription pad or letterhead indicating your inability to serve. We do not need to know the type of condition or any medical information, only that you are unable to fulfill the requirements of jury service. The doctor’s note should be attached to your affidavit requesting excusal (see previous question).
